If I have full insurance coverage of my vehicle and my sister who is 30 and does not live with me wants to borrow my car every now and then maybe once a month at most do I have to have additional insurance to cover her?

If your sister drives your car regularly, then she should be listed on the policy.
If it is not on a regular basis, you can lend it out to her. Assuming she is a licensed driver. And you are willing to accept responsibility for any claims / accidents. (ie a big insurance increase if she has an accident)
